Many access failures begin with a reasonable decision that was never revisited. A person changes roles. A temporary exception becomes permanent. A service account outlives its original use.
Identity and access management therefore cannot stop at account creation. It must connect identity lifecycle, authentication, authorization, privilege and review.
Separate the layers
When access fails, teams often treat every problem as a password issue. A disciplined investigation asks whether the identity is known, authentication succeeded, policy allowed the action, and the resource interpreted that policy correctly.
Make ownership visible
Every exception should have a purpose, an owner and an end condition. Without those three elements, convenience quietly becomes risk.
